What is the role of a case manager/consultant?
The role of case manager is to collaborate with clients by assessing, facilitating, planning and advocating for health needs on an individual basis.
1. Assessor -The case manager gathers all relevant data and obtains information by interviewing client/family and performing careful evaluation of the entire situation.
2. Planner -The case manager works with client/family to develop a treatment plan which enhances client outcome and reduces the payor’s liability. The planning process includes the client/family as the primary decision maker and goal setter. Contingency plans are incorporated for each step in the process to anticipate treatment and service complications. The case manager initiates and implements plan modifications as necessary through monitoring and re-evaluation to accommodate changes in treatment or progress.
3. Facilitator -The case manager actively promotes communication between all team members, client, family, provider and all parties involved. Collaboration between the client and health team is enhanced, which maximizes outcomes. The health delivery process is streamlined to focus on the best treatment or approach for the client, thus minimizing or eliminating unnecessary steps and promoting the timely provision of care and utilization of resources. This is accomplished via coordination of the delivery of services.
4. Advocate -The client’s individualized needs and goals are incorporated throughout the case management process. The client is supported and educated to become empowered and self-reliant in self advocacy. The consensus of all parties is obtained to achieve optimal outcomes. Early referral is promoted to provide optimum care and cost containment. The client’s best interests are represented by the case manager’s advocacy for necessary funding, treatment alternatives, timelines and coordination of health services, and frequent re-evaluation of progress and goals.
