How Case Management Works

Case Management helps individuals reach an optimum level of wellness and functional capacity after sustaining catastrophic injuries from a car accident. Our experienced Case Managers are trained to provide detailed, and individualized solutions for rehabilitation and recovery. The process begins by pairing a Case Manager with the family. The Case Manager then conducts a thorough assessment of a person’s current status, history, and assessment of medical records to develop a plan with recommendations. Our Case Managers have a deep understanding of resources and barriers that may exist throughout the continuum of services.

What Can a Case Manager Provide?

Client Advocacy

How can an Educational Consultant help?

An Educational Consultant will navigate through the systems, meetings, and professionals involved with your child’s education. They advocate appropriate services for your child at school and align the family’s goals across all disciplines and service providers. The Educational Consultant offers a successful transition back to school after an accident, injury or diagnosis and will monitor student’s progress and adjust planning as curriculum changes with age and ability (help with an IEP).

Vocational Consulting

After a catastrophic injury, our Vocational Rehabilitation Counselor will ease your transition back to work, assessing career options and identifying current skills relevant to the job market. Together we will find a path that matches your career goals.

  • Assessment

    Our Vocational Rehabilitation Counselor will provide a detailed assessment to obtain information regarding past work history, restrictions, interest, abilities, and career options.

  • Direction

    Working with physicians and community resources, our Vocational Rehabilitation Counselor assists with resume development, finding job leads, and aids with employment applications and the interview process.

  • Social Security Disability Insurance (SSDI)

    The amount of paperwork associated with an SSDI claim is both time-consuming and confusing. Our Vocational Rehabilitation Counselor can assist you with completing all of the appropriate paper work as well as mentor you through the SSDI interview and claims process.

OT Assessments

When it is finally time to leave the hospital, nothing sounds better than going home and sleeping in your own bed. However, going home after suffering a catastrophic injury can present a new set of physical challenges. We can help. Our creative and knowledgeable Occupational Therapist specializes in home accessibility and safety evaluations. Even before you leave the hospital, the therapist will begin the process of assessing your home to create a safe environment for your return.

Recommendations can include:

  • Equipment or structural modifications to make your new home life easier and safer for you and your family.
  • A ramp to bypass the front entry stairs
  • Grab bars to steady your balance when entering the bathtub or a specialized shower chair to fit that unusual shower stall.

Down the road, if your injuries dictate extensive home modifications, the occupational therapist will conduct a comprehensive evaluation of your home, abilities, and goals.

 

Working side-by-side with you and building professionals, your home can be redesigned to support independence and moving forward in your life.

Case Management for Substance Use Disorder

After sustaining a catastrophic injury resulting from a car accident, injured individuals are prescribed various opioids to relieve pain. Regular or long-term use of opioids can lead to physical dependence, which can often result in substance misuse, and hospital readmission.

We have over 21 years of case management experience coordinating care for individuals who have sustained catastrophic injuries.
