How Case Management Works
Case Management helps individuals reach an optimum level of wellness and functional capacity after sustaining catastrophic injuries from a car accident. Our experienced Case Managers are trained to provide detailed, and individualized solutions for rehabilitation and recovery. The process begins by pairing a Case Manager with the family. The Case Manager then conducts a thorough assessment of a person’s current status, history, and assessment of medical records to develop a plan with recommendations. Our Case Managers have a deep understanding of resources and barriers that may exist throughout the continuum of services.
Case Management focuses on helping individuals reach an optimum level of wellness and functional capability while creating purpose in their lives. Case Management serves to create a means for achieving client wellness and autonomy through advocacy, communication, education, identification of service resources, and service facilitation.
The Case Manager identifies appropriate providers and facilities throughout the continuum of services. Resources are utilized in a timely and cost-effective manner to obtain optimum value for the client and the reimbursement source.

Catastrophic Case Management
What Can a Case Manager Provide?
Having a professional on your side means equal footing when it comes to big decisions. Our clients have years of experience behind them with Feinberg Consulting’s advocacy. A case manager designs a consolidated and detailed plan, utilizing all resources available.
Our collective experience and decades of service has allowed us to compile a database of trusted resources and strong relationships. Our case managers will identify the specific resources that will allow clients to make informed decisions.
There is life beyond the hospital or rehabilitation facility, and it can be difficult at first. The last thing anyone wants to deal with is additional setbacks. Our case managers create a plan for transition, which is inline with clients’ recovery goals.
The Coach of Your Care Team
A case manager brings unity to the treatment team, collaborating with: physicians, other rehabilitation providers, attorneys, insurance adjusters and clients’ families. The case manager creates a central hub through which all of the information is gathered and maintained – clients have a “go to” person.
With early intervention the case manager will coordinate a plan that reduces duplication of services, while receiving optimal care for greater outcomes. Our case managers work to prevent duplication of services, setbacks, and ultimately create greater outcomes that are cost efficient.

When it is finally time to leave the hospital, nothing sounds better than going home and sleeping in your own bed. However, going home after suffering a catastrophic injury can present a new set of physical challenges. We can help. Our creative and knowledgeable Occupational Therapist specializes in home accessibility and safety evaluations. Even before you leave the hospital, the therapist will begin the process of assessing your home to create a safe environment for your return.
Recommendations can include:
- Equipment or structural modifications to make your new home life easier and safer for you and your family.
- A ramp to bypass the front entry stairs
- Grab bars to steady your balance when entering the bathtub or a specialized shower chair to fit that unusual shower stall.
Down the road, if your injuries dictate extensive home modifications, the occupational therapist will conduct a comprehensive evaluation of your home, abilities, and goals.
Case Management for Substance Use Disorder
After sustaining a catastrophic injury resulting from a car accident, injured parties are prescribed various opioids to relieve pain. Regular or long-term abuse of opioids can lead to physical dependence, which can often result in addiction, and readmission.
Substance abuse or misuse impairs healing and recovery. Research indicates that when addiction is treated as a chronic medical condition, and long-term treatment and recovery care are provided, it leads to significant reductions in overall healthcare costs.
We have 20 years of Case Management experience coordinating care for individuals who have sustained catastrophic injuries. We specialize in the coordination and management of care for those who have an acquired substance use disorder. Our team of trusted and compassionate professionals are committed to supporting clients in healing and long-term recovery.
