Initial Consultation
Initial Consultation with the elderly individual and/or family members regarding available services and options.

 

Assessment
In-Person Assessment of the elderly individual relevant to their health and living environment. This may include, but is not limited to:

 

Care Manager Assessment

  • Review of health status and medical needs
  • Review of environment and support system
  • Functional analysis
  • Assessment of nutritional needs
  • Identification of safety issues
  • Physical and mental limitations
  • Review of overall quality of life

 

Develop Care Plan
Development of written care management recommendations to meet identified needs, as well as accommodate the entire family. Care Plan addresses the results of the assessment. Care plan includes, but is not limited to:

 

Care Plan

  • Recommendations of best housing option
  • Connection to community resources
  • Recommendation of best care option
  • Links to transportation, legal assistance, financial services, home modification, counseling, etc.

 

On-going Elder Care Management
To help set up, monitor, and coordinate services. This may include, but is not limited to:

 

Care Management

  • Maintaining records, including physician appointments
  • Monitoring elder’s capabilities and functions
  • Communication with family members
  • Acting as elder’s advocate with family, physician, home health and/or other provider
