- Is the Care Manager licensed in their profession?
- What additional certifications does the Care Manager have? i.e.: Case Manager Certified (CMC), Certified Case Manager (CCM), Certified Advanced Social Work Case Manager (C-ASWCM), Certified Social Work Case Manager (C-SWCM).
- Length Care Manager has provided Case Management services?
- How is the Care Manager going to communicate and provide reports as needed?
- What are the Care Manager’s fees and can I receive them in writing?
