Goals of Care Management

  • Through early assessment, ensure that services are generated in a time and cost-effective manner.
  • Assist clients to achieve an optimal level of wellness and function by facilitating timely and appropriate health services.
  • Assist clients to appropriately self direct care, self advocate and make decisions to the degree possible. Maintain cost effectiveness in the provision of health services.
  • Maintain dignity for the client in the most appropriate setting ensuring safety and minimizing risks.
  • Foster self-determination to enable the client to live in accordance with his or her personal values and goals.

 

Questions a family may ask when hiring a Care Manager:

 

  • Is the Care Manager licensed in their profession?
  • What additional certifications does the Care Manager have? i.e.: Case Manager Certified (CMC), Certified Case Manager (CCM), Certified Advanced Social Work Case Manager (C-ASWCM), Certified Social Work Case Manager (C-SWCM).
  • Length Care Manager has provided Case Management services?
  • How is the Care Manager going to communicate and provide reports as needed?
  • What are the Care Manager’s fees and can I receive them in writing?
  • Can I obtain references?
