Our mission
To assist families struggling with the needs of aging relatives by providing services enabling the elderly to live independently in a safe environment, without sacrificing their dignity, respect, or quality of life; and to give families a peace of mind and a support system they can rely on.
Overview
Our Elder Care Management Program offers services to families and professionals who are caring for an older adult, through assessment, plan of care development, and on-going monitoring process. Individualized professional consultation provides families and professionals with tangible solutions, resources and care options.

Feinberg Consulting assures you that each client will be evaluated as an individual. Our team of Nurses, Social Workers, Educational Consultants, Licensed Professional Counselor and Office Staff will devise a plan specific to meet the needs of every client.
Questions families need to ask themselves to determine if a Care Manager is needed:
Are the problems that you or your loved ones are facing becoming larger and more complex than you can comfortably manage? Are other demands and responsibilities now so great that you are not able to provide the desired level of supervision and attention to your loved one’s problems?
Purpose of Care Management
To interject objectivity and provide information where it is lacking. To maximize efficiency in utilization of available resources. To work collaboratively with patient, physician, family, attorney, significant other and provider of healthcare to implement a plan of care which meets the individual’s needs. To ensure the utmost quality of care is provided utilizing the available financial resources to include: Medicaid, private health insurance, long-term care insurance, and private pay.
Services
Our program is a services model for the care of the elderly. Our professionals look at the whole picture by:
1. Initial Consultation with the elderly individual and/or family members regarding available services and options.
2. In-Person Assessment of the elderly individual relevant to their health and living environment. This may include, but is not limited to:
3. Development of written care management recommendations to meet identified needs, as well as accommodate the entire family. Development of care plan will address the results of the assessment. Care plan includes, but is not limited to:
4. On-going care management services to help set up, monitor, and coordinate services. This may include, but is not limited to:
Feinberg Consulting’s licensed Professional Counselor can provide compassionate support to individuals and family members to help them cope with challenges during times of crisis. Crisis has an effect on the individual and the family dynamic. A counselor can help guide and improve lines of communication to allow the best outcome for both the client and their family.
An Occupational Therapist can help individuals who suffer from a mentally, physically, developmentally, or emotionally disabling condition to develop, recover, or maintain their daily living skills. The therapist can recommend activities or equipment that not only improves basic motor functions and reasoning abilities, but also to compensate for permanent loss of function. An OT functional home evaluation will evaluate the home environment and function of individuals to assess for home adaptive aides, equipment and home modifications needed to assure a safe environment. The goal is to help clients have independent, productive, satisfying lives.
